Five major companies launched dedicated health AI agents between January and March 2026, and all five follow the same design: connect your electronic health records, wearables, and wellness apps into one personal hub, then answer questions against the result. None of them is a doctor replacement, and the differences between them are far smaller than their marketing suggests — all five offer no-training commitments and separate health memory, and none offers the strongest available privacy protections.

Here is what the category actually is, how the five compare, and the specific things you should decide before connecting a single record.

What changed in 2026

For years, health AI meant typing symptoms into a general chatbot. According to OpenAI, around 230 million people ask ChatGPT health questions every week, and over 40 million do so daily. Those conversations ran under ordinary chat privacy defaults, in the same memory space as a request to draft a birthday card.

The 2026 shift was structural: the major providers split health out into separate products with separate storage. The IAPP's analysis of the five launches documents the timeline and finds that every one of them converges on the same architecture — records plus wearables plus a promise not to train on your data.

The five contenders, compared

All five are US-only for now. All five connect to electronic health records through third-party intermediaries rather than direct integrations.

The intermediaries matter more than the brands. Your records generally reach the agent through a service such as b.well, HealthEx, or a state health information exchange — which means a third party is in the path of your medical history, and its terms are as important as the agent's.

What all five get right

Two commitments are genuinely meaningful and appear across the category.

  • No training on health data. Health conversations are not used to improve the models. This is a real departure from the default for general chat.
  • Isolated health memory. Health conversations are stored separately from ordinary chats, so a question about a medication does not surface in unrelated sessions.

Both are worth having. Neither is the same as being unable to read your data.

What none of them give you

The IAPP comparison found the same gap in every product: none offers cryptographic hardening strong enough to constrain the provider's own ability to read your health conversations in plaintext. That matters in a specific, concrete scenario — if the data is demanded through civil discovery or a government request. It is no longer hypothetical: a court order in the consolidated copyright litigation required OpenAI to produce millions of ChatGPT conversation logs.

Three further limits apply across the category:

  1. Human access rules are vague or undocumented. Four of the five do not clearly state who inside the company can read your health conversations, and under what process.
  2. Governance is unilateral. Each provider sets and changes its own framework without independent verification.
  3. Europe is excluded entirely. Every product bundles privacy protections with data integration, and it is the integration that triggers GDPR Article 9 requirements, medical device rules, and probable AI Act high-risk classification. Europeans therefore get neither the records connection nor the stronger privacy defaults already sitting on the same platform.

What doctors actually think

The clinical community is not broadly opposed, and the nuance is instructive. The American Medical Association's 2026 Physician Survey found 81% of US physicians now use AI in their practice. Majorities are comfortable with patients using AI for medication questions (68%) and general health queries (64%) — but roughly half oppose patients using AI to interpret pathology (49%) or radiology (46%) results.

Read that as a line drawn inside the category: use it to understand and prepare, not to diagnose from a scan.

  2. Read the intermediary's terms. Not the agent's. If b.well or HealthEx sits in the path, their retention and sharing rules govern your data too.
  3. Decide what never goes in. Genetic results, mental health notes, and anything you would not want produced in litigation are reasonable exclusions regardless of the provider's promises.
  4. Keep your own copy. The patient portal and a local archive should hold the authoritative record, not the agent's memory.
  5. Verify before acting. Ask the agent for the source of any clinical claim, and check the number against the document it cites.

The local-first argument here is narrower than it is for most tasks, because a health agent's whole value is the data connection — and a machine you own cannot read records the hospital will not release to it. What it can do is hold everything downstream: the plain-language explanation of a lab result, the list of questions for your next appointment, the chronology you build across years of PDFs.
